12-18. Diffuse axonal injury (DAI) is the most
important mechanism producing long-term neuro-
logical de¬cits after closed head injury. DAI is sec-
ondary to axonal shearing that occurs from acceler-
ative-decelerative forces on the brain during head
trauma. A. Coronal section at the level of the hypo-
thalamus shows shrinkage of the hypothalamus and
enlargement of the third ventricle. There is discol-
oration of the fornices and the mamillary bodies
(MB). The parasagittal white matter also appears

12-19. Huntington disease (HD) is an autosomal
dominantly inherited neurologic disease character-
ized by chorea and cognitive decline. HD is the
most common form of neurodegenerative disease
due to an expansion of a CAG trinucleotide repeat
in the affected gene (on chromosome 4p16.3) that
codes for a novel protein (huntingtin). A. Un¬xed

12-20. Progressive multifocal leukoencephalopathy
(PML) is a fatal opportunistic viral infection caused
by the JC virus in oligodendrocytes that presents as
a subacute demyelinating disease. It may also occur
in patients with HIV disease. A. Gross appearance

plaques are not as discrete as those of multiple scle-

12-27. Arteriovenous malformation (AVM) of the brain is a congeni-
tal abnormality comprising multiple direct connections between
arteries and veins with no intervening capillary circulation. Symp-
toms may develop later in life due to rupture or pressure on brain
tissue, seizures may occur, or even heart failure may occur, mimick-
ing dilated cardiomyopathy if the shunt is large. A. Surgically
removed AVM shows brain tissue containing blood vessels having a
“bag of worms” appearance due to dilatation and elongation second-
ary to the greatly increased ¬‚ow resulting from high arterial pressure
leading directly into low venous pressure vessels. Endovascular treat-
ment or radiosurgery may be used to treat AVM. B. Histology of the
lesion shows a mixture of dilated arteries and veins coursing through
the cerebral tissue. C. AVM surgically removed from the spinal cord.

12-28. An encephalocele occurs when the neural
tube fails to close completely, resulting in portions
of the brain and its membranes protruding from the
skull in a sac-like formation. Like spina bi¬da, this
“cranium bi¬da” may be linked to maternal folic
acid de¬ciency during pregnancy (among other
causes). A. Posterior encephalomeningocele is ele-
vating the skin over the lower back of the head. B.
Sac comprising brain and membranes is present
(arrow), just deep to the re¬‚ected scalp. C. Follow-

12-29. Anencephaly is another neural tube anom-
aly. It is characterized by the absence of scalp, cal-
varium, and normal brain, which is substituted by
an angiomatous mass. This ¬gure shows a bisected
newborn with anencephaly. The upper spinal cord
ends at the site of maldevelopment of the anterior
end of the neural tube.




12-30. Holoprosencephaly, a developmental ¬eld
defect of impaired midline cleavage of the embry-
onic forebrain in which the facial appearance pre-
dicts the brain anatomy in about 80% of cases, may
show a range of appearances. The most extreme
form is cyclopia in which there is a single midline
eye. The illustrated fetus shows two closely set
eyes, arhinia, and a proboscis formation on the fore-
head. The brain had failed to cleave into two cere-
bral hemispheres, and no olfactory tracts or bulbs
were present.
